We welcome your vision insurance here at Comfort Vision. Thank you for selecting us for your eye care needs. Our staff will be glad to check on your available benefits. We accept most vision insurance plans available to Washington State residents including:

- Regence/Blue Shield
- Premera Blue Cross
- Eyemed
- Aetna
- United Healthcare
- Medicare Part B
- And many more!

We want to minimize any problems that you may have with your insurance plans and payments. We are happy to help you receive the maximum benefits that you are allowed for your vision coverage. In order to submit your vision insurance, we will need all the information filled out on the insurance portion of the health questionnaire form.

Here is a general overview of our insurance filing policy:

-Participating Health Care Plans: For the insurance plans that we participate in, you are only responsible for any applicable co-payment during your visit and for any services that are not covered by your insurance. Please check with your insurance company to confirm that we are participating/preferred providers before you schedule your appointment. If your plan requires an authorization form filled by your primary care provider, please bring this completed form to our office at the time of your appointment, otherwise you may be financially responsible for any applicable charges.

-Nonparticipating Health Care Plans: For the insurance plans that we are not participating in, we will be happy to file a claim to your insurance company for you or to provide you with the necessary paperwork for you to submit the claim on your own, but you are responsible for all the balance at the time of service. Any reimbursement will be sent directly to you after your claim has been processed.

Our goal through your whole exam, including diagnosis and treatment phases, is to provide you with the best possible vision care. We do not allow the insurance companies tell us how to treat you. Our recommendations are based on your needs and what will provide the best outcome for you. Feel free to call us for any questions that you may have!

Please fill out the patient health history form and sign the privacy policy at the bottom and give this form to our staff at the time of your appointment. This can save you time during your office visit, which may last up to 60 minutes during the first visit.
